Case 5: The Case of the Fixed Pupil

A 72-year-old white female presented with a chief complaint of blurry vision OS since trabeculectomy performed the previous week.

The Clues

  • Hypertension
  • Primary open angle glaucoma (POAG) for 7 years
  • Recent history of ALT-induced pressure spike, non-responsive to intense medical therapy
  • BVA 20/20 OD, hand motion at 5 feet OS
  • Fixed pupil OS
  • IOP 18 mmHg OD and 02 mmHg OS
  • Slit lamp exam revealed flat anterior chamber OS, 2+ nuclear sclerosis OU. No leak at bleb site OS
  • Fundus OD remarkable only for glaucomatous optic nerve head changes; fundus OS as shown below
