You may call or e-mail us for an eye examination appointment.
Appointments are not necessary to visit our eyewear center, but please be sure to check in at the main desk when you arrive.
Patient Information and History Forms
To facilitate your check-in process when you come in for your appointment, you are welcome to download, print, and fill out the forms ahead of time. When do you need to fill out the forms?
- If you are a new patient
- If your personal information (address/phone number/insurance) has changed since the last visit
- If your medical history/medication has changed since the last visit
- If it has been three years or longer since the last visit
