	Office Hours Monday-Friday 8:00am to 5:00pm The clinic will be closed on Monday, January 18, in observance of Martin Luther King, Jr. Day.  Normal hours will resume on Tuesday. The clinic will be closed on Saturday, April 3, 2010.
	Appointments To make an eye examination appointment, please call us at 317-321-1470. Appointments are not necessary to visit our eyewear center, but please be sure to check in at the main desk when you arrive. Patient Information and History Forms To facilitate your check-in process when you come in for your appointment, you are welcome to download, print, and fill out the forms ahead of time. When do you need to fill out the forms?
