	Appointments To make an eye examination appointment, please call us at 317-321-1470. Appointments are not necessary to visit our eyewear center, but please be sure to check in at the main desk when you arrive. Patient Information and History Forms To facilitate your check-in process when you come in for your appointment, you are welcome to download, print, and fill out the forms ahead of time. When do you need to fill out the forms? If you are a new patient If your personal information (address/phone number/insurance) has changed since the last visit If your medical history/medication has changed since the last visit If it has been three years or longer since the last visit The forms: General patient information form Medical history form (13 years and older) Medical history form (12 years and younger) For your information, IU School of Optometry notice of privacy practices IU School of Optometry eye care centers are offering a 10% discount on spectacle frames and lenses for IU employees and students.
