You must apply through OptomCAS AND Indiana University’s Apply Yourself system.

(requires $55 supplemental application fee)

On Indiana University’s Apply Yourself application, you are only required to fill out the following sections:

  • Personal Information
  • Educational Objectives
  • Indiana Residency

PLEASE NOTE: For letters of recommendation, the Apply Yourself system requires that you enter two recommender names.  Please enter two recommender names but, make sure to check “Request Offline” so the system will not email your recommenders.  We must receive all your letters of recommendation through OptomCAS.

You do NOT need to fill out the following Apply Yourself Application sections:

  • Educational History
  • Test Scores
  • Work Experience
  • Publications
  • Personal Statement
You must also fill out the Optometry Departmental Questions, which includes a pre-requisite worksheet and a short essay section.  There is a link at the end of the Apply Yourself application or you can find it here: https://www.indiana.edu/~grdforms/deptforms/optb.html

Indiana University Application Checklist:

  • Completed OptomCAS application
  • ApplyYourself application with required sections completed
  • Optometry Departmental Questions, which includes the essay and prerequisite worksheet.
